For the FNP boards, montelukast is an add-on controller for persistent asthma and a second-line option for allergic rhinitis. It blocks the cysteinyl leukotriene CysLT1 receptor, reducing eosinophilic airway inflammation and bronchoconstriction. Indications include persistent asthma (add-on to ICS rather than substitute), exercise-induced bronchospasm prophylaxis, and allergic rhinitis. The 2020 FDA black box warns of serious neuropsychiatric effects — agitation, depression, suicidal ideation, sleep disturbance — particularly in children and adolescents. The board reflex is to reserve montelukast for patients who cannot tolerate or use an inhaler and to counsel families on watching for mood changes. Once-daily dosing in the evening; chewable for younger children.

10 mg PO daily in evening. Peds 6–14: 5 mg. Peds 2–5: 4 mg.

Severe hepatic impairment, hypersensitivity.
