The NP-board angle on spironolactone is HFrEF mortality benefit — alongside ACE/ARB and a beta-blocker, it is one of the four pillars of guideline-directed medical therapy. It antagonizes the mineralocorticoid receptor, sparing potassium and reducing remodeling. Indications include HFrEF (mortality benefit), resistant hypertension as a fourth-line agent, primary hyperaldosteronism, cirrhotic ascites, and off-label acne and hirsutism in women. The signature side effects are hyperkalemia (especially with ACE/ARB or potassium supplements) and gynecomastia and breast tenderness from anti-androgenic activity — eplerenone is the cleaner alternative when gynecomastia is a problem. Monitor potassium and creatinine 1–2 weeks after starting and after dose changes.

🚫 Contraindications
Hyperkalemia (>5.0), Cr >2.5 (men) or >2.0 (women), severe CKD, Addison disease.

K⁺ and creatinine within 1 week of start, then monthly × 3, then periodic.
