Sertraline

For the FNP boards, sertraline is the SSRI with the broadest FDA-approved indication list and the safest pregnancy profile in its class. It treats major depression, generalized anxiety, panic disorder, OCD, PTSD, social anxiety, and premenstrual dysphoric disorder. Onset of antidepressant effect is 4–6 weeks; GI side effects (nausea, diarrhea) appear in days and usually wane. Sertraline has fewer drug interactions than fluoxetine or paroxetine and is preferred in pregnancy and breastfeeding when an SSRI is needed. Watch for serotonin syndrome with tramadol, MAOIs, linezolid, or triptans. Taper to avoid discontinuation syndrome. Sexual dysfunction is the chronic side effect that drives switches in adherent patients.

🚫 Contraindications
MAOI use (within 14 days), concurrent pimozide, true SSRI allergy.
⚠️ Adverse Effects
Sexual dysfunction, GI (N/D), insomnia, serotonin syndrome, discontinuation syndrome, ↑ suicidal ideation (<24 yrs, black box), hyponatremia (SIADH), bleeding risk.
