The NP-board angle on doxycycline is recognizing the syndromes where it is empiric and the patients in whom it is forbidden. As a tetracycline, it inhibits the 30S ribosomal subunit and covers atypical pneumonia (Mycoplasma, Chlamydophila), tick-borne illness (Rocky Mountain spotted fever, Lyme, ehrlichiosis, anaplasmosis), CA-MRSA skin infections, chlamydia, acne vulgaris, and malaria prophylaxis. For RMSF, start doxycycline before serology returns — delay kills. Avoid in pregnancy (developmental tooth and bone effects) and in children under 8 (tooth staining), with the RMSF exception in pediatrics where the benefit outweighs the risk. Take with a full glass of water, sit upright 30 minutes — pill-induced esophagitis is real.

100 mg PO BID × 7–14 days (typical). Acne: 50–100 mg daily.

Pregnancy (all trimesters), children <8 yrs (tooth staining + bone growth), true tetracycline allergy.

☀️ Photosensitivity (counsel on sunscreen), GI upset, pill esophagitis (take with water, upright), tooth staining (<8 yr old).
