Full name: Mean Corpuscular Hemoglobin Concentration
MCHC, or mean corpuscular hemoglobin concentration, measures the average concentration of hemoglobin inside a given volume of red blood cells. Unlike MCH, which reflects hemoglobin content per cell, MCHC accounts for cell size and indicates how densely packed hemoglobin is within erythrocytes. Clinicians use it alongside MCV and RDW to classify anemias as hypochromic, normochromic, or hyperchromic.

Low MCHC signals hypochromic anemia and points to iron deficiency, thalassemia, chronic disease, or sideroblastic anemia. Elevated MCHC is less common and raises suspicion for hereditary spherocytosis, autoimmune hemolytic anemia, or severe burns that damage red cell membranes. Spuriously high values result from cold agglutinins, lipemia, or hemolyzed specimens, and a repeat sample clarifies the picture before further workup.
On the AANP exam, expect MCHC primarily in anemia classification questions. Expect vignettes pairing a low MCHC with microcytosis to test recognition of iron deficiency anemia, and scenarios linking elevated MCHC with jaundice and splenomegaly to identify hereditary spherocytosis. Candidates need to distinguish MCHC from MCH, recognize normochromic patterns in acute blood loss and anemia of chronic disease, and select iron studies as the next diagnostic step when microcytic hypochromic indices appear on a CBC.
