Hemoglobin trends also help monitor response to treatment in chronic kidney disease, GI bleeding, and pregnancy.
Decreased hemoglobin points to iron deficiency, chronic blood loss, thalassemia, anemia of chronic disease, B12 or folate deficiency, hemolysis, bone marrow suppression, and renal failure from reduced erythropoietin. Elevated hemoglobin reflects dehydration, chronic hypoxia from COPD or sleep apnea, high-altitude residence, smoking, testosterone therapy, and polycythemia vera. NPs pair hemoglobin with MCV, RDW, and reticulocyte count to classify anemia before ordering iron studies or a peripheral smear.
On the AANP exam, expect recognition of sex-specific reference ranges, the hemoglobin threshold that defines anemia in pregnancy, and first-line workup for microcytic versus macrocytic anemia. Expect questions that pair hemoglobin with MCV to identify iron deficiency, thalassemia, or B12 deficiency. Items also test when to refer for suspected polycythemia vera and when to evaluate for occult GI bleeding in older adults with unexplained drops in hemoglobin.
