Free T4 measures the unbound, biologically active fraction of thyroxine circulating in the blood. Unlike total T4, it reflects hormone available to tissues and remains unaffected by changes in thyroid-binding globulin from pregnancy, estrogen therapy, or liver disease. NPs pair Free T4 with TSH to confirm thyroid status and pinpoint whether dysfunction originates in the pituitary or the thyroid gland itself.

Clinical Context

Elevated Free T4 points to Graves disease, toxic multinodular goiter, thyroiditis in its early inflammatory phase, excess levothyroxine replacement, or amiodarone-induced thyrotoxicosis. Depressed Free T4 signals primary hypothyroidism from Hashimoto thyroiditis, post-ablation or post-thyroidectomy states, severe iodine deficiency, or central hypothyroidism from pituitary disease. Pregnancy, critical illness, and certain medications shift values and require careful correlation with TSH.

High-yield for NP boards: the pairing pattern — high TSH with low Free T4 confirms primary hypothyroidism, low TSH with high Free T4 confirms primary hyperthyroidism, and low TSH with normal Free T4 suggests subclinical hyperthyroidism.
