Folate, also called vitamin B9, is a water-soluble vitamin essential for DNA synthesis, red blood cell production, and methylation reactions. Serum folate reflects recent dietary intake, while RBC folate indicates tissue stores over the prior three to four months. Adequate folate supports normal erythropoiesis and fetal neural tube development during pregnancy.

Clinical Context
Folate, also called vitamin B9, is a water-soluble vitamin essential for DNA synthesis, red blood cell production, and methylation reactions. Serum folate reflects recent dietary intake, while RBC folate indicates tissue stores over the prior three to four months. Adequate folate supports normal erythropoiesis and fetal neural tube development during pregnancy.
Depressed folate levels stem from poor dietary intake, chronic alcohol use, malabsorption syndromes such as celiac or Crohn disease, and medications including methotrexate, phenytoin, trimethoprim, and sulfasalazine. Pregnancy and hemolytic anemias increase demand and drive levels down. Deficiency produces megaloblastic macrocytic anemia with an elevated MCV, hypersegmented neutrophils, and glossitis. Elevated folate carries little clinical weight but masks the hematologic signs of B12 deficiency while allowing neurologic damage to progress.
What the AANP exam actually tests on folate: the link between deficiency and macrocytic anemia, and expects candidates to distinguish folate from B12 deficiency by the absence of neurologic symptoms in folate deficiency. Questions emphasize preconception and first-trimester supplementation of 400 to 800 mcg daily to prevent neural tube defects, higher doses for patients on anticonvulsants or with a prior affected pregnancy, and the requirement to check B12 before treating macrocytic anemia with folate alone.
